Disability Support

Our specialist outreach disability support service aims to empower people with complex needs who feel they have struggled, or have been unable, to access services and/ or facilities because of their disability. Sometimes a disability can make it harder to get people to listen to you and take what you say seriously.

Our service supports people who may need information and/ or support to resolve an issue. Perhaps you are dealing with complex situations, requiring the coordination of a number of services to resolve. Also, we can assist you if you have been discriminated against because of your disability/ies.

The support provided includes and is not limited to:

  • Providing information on rights, entitlements, and ensure those rights and entitlements are upheld
  • Exploring options and assist people to make decisions and provide the support needed to take those decisions forward
  • Helping disabled people navigate Care and Health Pathways and communicate their needs
  • Attending meetings with other professionals
  • Supporting clients to prepare for, part take, understand and appeal against decisions made in Care Needs Assessments
  • Researching information to support clients understand their rights and entailment and explore their options
  • Signposting and supporting clients gain access to different services.

The work is carried out on a case by case basis. Please bear in mind that there is a waiting list.
